Hockey Queensland appoints Assistant Tournament Directors, Technical Managers and Assistant Technical Managers to various State Championships.  

HQ ATD/TM/ATM Feedback Form provides Hockey Queensland with information on our appointed Assistant Tournament Director, Technical Manager or Assistant Technical Manager and their performance at each event they manage. This information allows Hockey Queensland to identify areas of improvement and successes for our Assistant Tournament Directors, Technical Managers and Assistant Technical Managers. 

You will need to submit a new form for each Tournament Official you are providing feedback on.

The form is sent to relevant stakeholders who have a direct working relationship with the appointed Assistant Tournament Director, Technical Manager or Assistant Technical Manager. These identified stakeholders include Assistant Tournament Directors, Technical Managers, Assistant Technical Managers, Technical Mentors, Technical Assessors, Umpire Managers, Umpire Coaches and Technical Officials and HQ staff for all HQ State Championships and/or HQ Events.  

The HQ ATD/TM/ATM Feedback Form information will be kept on file with Hockey Queensland Office and used as part of the appointment process for future HQ State Championships.  All feedback submissions will be collated and provided to individuals.  All submissions are anonymous, however to access the Feedback form you will need to log in with your RevSPORT Member details as the form can only be accessed by Current registered members of HQ.
