Hockey Australia, in partnership with Hockey Queensland and the other states, is conducting a nation-wide survey for our members.
Whether you are a new player, a returning player, or haven't taken to the pitch in 2020, your feedback on your hockey experience is important to us.
We would value you taking 3 minutes to complete the survey below to help us improve for 2021.
Please complete the relevant survey for EACH member of your household that plays hockey. You can complete these surveys multiple times.

NEW MEMBER You didn't play hockey in 2019, but registered in 2020. You could be a brand new member, or someone returning after a break. |
