This form is to be completed if you wish to nominate for selection in a Queensland team to attend the 2020 Australian Women's Masters Championships in Cairns, to be held from 2nd to 11th July, 2020.

Player Committment

• Players selected in a Qld team, must be assembled in Cairns by 1.00pm on Monday, June 29.  Teams will be training on Monday afternoon.
• The National Championships will be held 6 weeks after the State Championships.  Consequently the following process wil be in place upon selection for Qld:

          - to confirm position in team players must pay $1,000.00 by Friday, May 29.

          - final invoices must be paid by Monday, June 15.

          - players must order uniforms by Friday, May 29.

          - accommodation has been booked up to Saturday, July 11.  Players my leave the continent following the Closing Ceromony.

Nomination Information

• The $30.00 nomination fee is non-refundable
• Late nominations, and nominations which do not include payment of the nomination fee, will not be accepted by Hockey Queensland under any circumstances
• All players who have nominated for State selection must wear a ribbon at all times whilst playing matches during the Championships. Ribbons must be visible and must not clash with Captain’s ribbons/bands
• Players seeking exemption must fill out  the nomination form, pay $30.00 and submit an exemption form
• Trials on Thursday, May 21 will be held for Over 45, Over 50, Over 55, Over 60 & Over 65 players and Goal Keepers of all ages.
• Trials begin at 12.30pm.  Goal Keeper trials will begin at 7.00pm
• Trials are not compulsory.  Players become eligible for Qld trialing and/or playing for an Association

Supplementary Nomination Process - Please note the following with regard to the ‘Supplementary Nominations’:
• This nomination category is for players who have been called into a team as a last minute emergency
• Supplementary Nominations must be completed online - the $30 nomination fee must be paid online at time of nomination
• Nominations can be submitted between 10.00am Monday, May 18 and 10.00am Wednesday, May 20, 2020.  No nomination will be considered after this time.
• A Supplementary Nomination will not be accepted from players who were included on Form 1 – Team Entry (submitted Monday, April 20, 2020)
• A Supplementary Nomination will only be accepted from players who are listed on a team’s Form 7 – Pre-Tournament Team List (due by 10.00am Wednesday, May 20, 2020).
