Some competitions require Proof of Vaccination status for anyone 16 years or older. This could be due to Government and or venue regulations and following health advice.

The following process has been developed by Hockey Queenlsand for required members to show proof of vaccination in a secure way for all required events in 2022. Hockey Queensland will require any submission from March 1 onward to display "up-to date" status of vacination. This means you must have received your second dose within the last three months of submission. If your second dose has been administered more than three months ago, you will have to show proof of a booster shot.

Please upload your Certificate/Proof of Vaccination below. Please ensure it contains the following information to be accepted:

full name
DOB
timestamp of the document
date when each dose was adminsitered

Once your certificate has been verified it will be deleted and you will be confirmed as vaccinated on our secure system so you do not have to upload it again.

It will only serve as a proof for eligibility to your role in the event and will not be shared to anyone outside the HQ office.
